Allahuma zawijnee li-man katabta lee fee umm ul-kitab `indak, bi-jaahi
Nabbiyik al-Mustafa (s).

Answer:
Walaikum assalam,
It means, “Oh Allah! Grant me to marry the one whom You wrote for me in the ‘Mother of Books’ with You, for the sake of Your Chosen Prophet (s).”
